Our History:
We are an amazing team of Home Healthcare Professionals with over 30 years of total experience in treating the homeless population along with indigent and low-income families. We have identified a severe shortage of respite facilities that can provide the appropriate amount of time for patients that need more care. We come from clinical and administrative backgrounds in healthcare, with this knowledge we will be able to provide a safe place for the patient to receive the treatment and care needed to prevent hospital readmission.
Our Vision:
While working with local hospitals, we have identified a significant number of underserved population readmissions. Post discharge from a hospital, the underserved population does not have a clinically conducive environment to support continued healing. Instead, they return to their prior living situation, often leading to readmission to the hospital for the same condition, some within a few hours or a few days.
As of January 2019, Colorado had an estimated 9,619 persons experiencing homelessness on any given day, as reported by Continuums of Care to the U.S. Department of Housing and Urban Development (HUD). Of that total, 719 were family households, 1,068 were Veterans, 545 were unaccompanied young adults (aged 18-24), and 2,187 were individuals experiencing chronic homelessness.
Our Mission:
The Foundation has been developed to support a situation in hospitals and throughout our community. We believe that working alongside the hospitals, we can help in altering the continuing care for our deeply underserved populations post hospital discharge. In lieu of returning to their previous situation which can lead to the deterioration of their conditions; the foundation would create the opportunity to reside in a facility with supportive surroundings that create an environment for continued healing.
Supportive conditions include: a bed, healthy food choices, and the professional on-going medical care that is required. Our mission is to help reduce the readmission rates to hospitals and to provide the underserved populations with continuing medical treatment; especially for those suffering from wounds and their complications, chronic diseases, medication management, and education. This dream takes a dedicated team to support those in need.
